Question:

A person form 15H submit to two banks compulsory.

Yes you have to submit form 15H to avoid TDS.

Banks have to deduct TDS regardless income of FD holder if interest amount is more than Rs. 40,000 (earlier Rs. 10,000) in a year.

So, to avoid TDS you should provide form 15H to both banks.
