Sir, My user id and passward i dont know and the mobile no.and Email which is metion by my consultant so difficult to get OTP for reset my user ID and Pass word kindly advice how to know my user id and pass on my mobile and e-mail ID

Ask you consultant your id and password. He has to provide the details.
Alternatively write to GST helpdesk and you can also file a complain against practitioners for using your property without your consent.
